DOUDNA FINE ARTS CENTER USE FORM

Thank you for your interest in the Doudna Fine Arts Center! This form should be used to request the use of facilities within the Doudna.

Requests must be submitted at least 2 weeks in advance of the event. Requests can not be submitted for events within two weeks of the submission date. For emergency requests, please contact the Doudna office.

Event requests are reviewed on a weekly basis. Please contact Dennis Malak, Director of Programming, Publicity, & Promotions at dfmalak2@eiu.edu or Conor Baltz, Director of Operations at clbaltz@eiu.edu with any questions.
